Teacher Dismissal Forms
OAH has a Request to Set form for Teacher Dismissal cases to collect additional information from the parties. Please use the form to request a hearing in all permanent certificated teacher dismissal cases.
A separate Request to Set form has been created for Motions for Immediate Reversal of Suspension (MIRS). Please use this form for all such motions.
All other motions shall be accompanied by a Teacher Dismissal Motion Cover Sheet. Failure to include a properly completed cover sheet may result in rejection of a motion.
All forms should be submitted to the Office of Administrative Hearings using the Secure E-File Transfer (SFT) system.

Teacher Dismissal Forms
Any party requesting a motion for immediate reversal of suspension shall complete the Request to Set: Motion for Immediate Reversal of Suspension form (OAH 17). Complete this form and submit it to your local Office of Administrative Hearings office.
The School District (Teacher Dismissal) Request to Set form (OAH 20) is for School Districts to request a teacher dismissal hearing for Commission on Professional Competence and Administrative Law Judge Only cases. Complete this form and submit it to your local Office of Administrative Hearings office.
The Teacher Dismissal Motion Cover Sheet form (OAH 22) is to be used with all teacher dismissal motions except continuances and MIRS. Complete this form and submit it to your local Office of Administrative Hearings office.
