File an Appeal on State Small Business and Disabled Veterans Business Enterprise Certification
Applicants that have received a certification denial, revocation or a suspension notification from state contracting can file an appeal on the decision.
The certification appeal process allows applicants to oppose the decision(s) regarding their firm's certification within 30 calendar days. Applicants or certified firms who would like to file an appeal of a decision, must follow the instructions below.
CERTIFICATION APPEAL INSTRUCTIONS

Step 1 - Select an Appeal Document
Select and complete the appropriate appeal document from the list of forms below.

Step 2 - Statement of Facts
Prepare a written statement of facts supporting the appeal request including supporting documentation.

Step 3 - Submit Appeal Package
Submit the completed appeal package within 30 calendar days from the date of the notification by either mail or fax to:
