Administrative Procedure Act Forms
On this page you will find a links to forms to use in State agency licensing cases under the Administrative Procedure Act (APA) hearing.
State Agencies will use these forms in hearings with OAH. All forms should be submitted using the OAH Secure E-File Transfer (SFT) system.

Request to Set Forms
The State Agency Request to Set form (OAH 13) State Agency Request to Set form is to be used by State Agencies to request a hearing. Please complete all sections of this form. Submit to your local Office of Administrative Hearings office along with required documents pursuant to the Cal.Code of Reg., tit.1 §1018. (Outside Link)

The Statement of Agency's Reasonable Efforts to Confer form (OAH 21) is to be competed and submitted with every Request to Set form in the event no dates were agreed upon, in accordance with the Cal.Code of Reg., tit.1 §1018. (Outside Link)
Continuance Forms
The Continuance for Unrepresented Respondents form (OAH 23) is designed to help self-represented parties provide information required when requesting a continuance of an APA hearing pursuant to the Cal. Code of Reg., tit.1 §1020. (Outside Link)
Complete this form and submit it to your local Office of Administrative Hearings office. All documents filed with OAH must include proof of service to show that the document or documents were served on OAH and all other parties involved in the matter. When filing documents with the General Jurisdiction Division, parties must comply with California Code of Regulations, title 1, section 1008. Many people use the CA Judicial Council Forms, which are accepted at OAH.
