Required DGS Statewide Form - GenAI Reporting & Factsheet Form (STD 1000) - 2303

The purpose of the GenAI Reporting & Factsheet Form (STD 1000) is to obtain critical information about the potential use of GenAI in state purchases of goods and services.

Bidders/offerors must submit a completed GenAI Reporting & Factsheet Form (STD 1000) at the time of their bid/offer to be deemed responsive. For detailed procedures, refer to section 2304.4 - Evaluation of Bids/Offers, Contract Award, and GenAI Disclosure, in this chapter.

Contractors are required to submit a completed GenAI Reporting & Factsheet Form (STD 1000) for any amendments to existing contracts so that risk can be assessed. For more information, refer to section 2305.3 – Amendments & GenAI Procurement Requirements in this chapter.

For transactions exempt from competition, a completed GenAI Reporting & Factsheet Form (STD 1000) is required prior to contract award to assess risk. For further steps, refer to section 2305.2 –Assessing GenAI prior to Contract Execution & Contracts Exempt from Competition, of this chapter.

GenAI Reporting & Factsheet Forms (STD 1000) are to be kept in the procurement file with bid/offer responses, and for transactions exempt from competition (e.g., proprietary software, contracts awarded based on an approved Non-Competitive Bid Justification, etc.).
